PERSONAL DATA PROCESSING CONSENT FORM
FOR ALTINBAŞ UNIVERSITY ALUMNI
I have received a “DATA PROTECTION PRIVACY NOTICE” from the data controller, drawn up on the basis of ‘Personal Data Protection Law of Turkey No. 6698’ (“Law”) and ‘Rules and Regulations Governing Data Protection Privacy Notices; I have clearly understood the reasons under which Altınbaş University (“Data Controller”) residing at Mahmutbey Dilmenler Caddesi, No:26 Bağcılar, İstanbul collects and processes my personal data, the methods the data controller uses to collect it, the purposes for processing my personal data, the legal basis of data processing, whether or not my personal data will be shared with third parties, if yes, with whom and for what purposes it will be shared, that I can exercise my rights about my personal data under Article 11 of the relevant law and how I can submit a query or request to the Data Controller as part of my rights.
The data controller will use the following methods (phone calls, surveys, graduate information forms) to collect, process and transfer my personal data, including my first and last name, Turkish Identification number, date of birth, nationality, email address, phone number, city of residence, country of residence, postal/residential address, name of my employer (company), sector, date of employment, education details, graduation year (personal details, contact details, location details, work details, education background) for the purposes of executing and supervising work activities, for communication and related purposes, (as explicitly stated in the privacy notice), and limited to these purposes only.
CONSENT: I hereby give my consent to the foregoing.
NON-CONSENT: I hereby do not give my consent to the foregoing.
